VitalCaring Group is seeking a dedicated Registered Nurse (RN) to provide in-home health care across an assigned territory. You will lead the plan of care, coordinate with physicians, and deliver skilled nursing services in patients' homes.

This field-based role offers clinical autonomy, strong team support, and opportunities to reduce hospitalizations through proactive care, education, and timely documentation.

Qualifikationen

• Active RN license (or compact) and CPR certification.
• Graduate of an accredited nursing program.
• Minimum 1 year RN experience in Home Health or related acute care.
• Strong clinical judgment and patient-centered communication.

Aufgaben

• Conduct in-home assessments to evaluate clinical status and goals.
• Develop and manage individualized care plans with physicians.
• Deliver skilled nursing care, including medications and therapies.
• Act as case manager coordinating therapists, aides, and physicians.
• Monitor progress and adjust care as conditions change.
• Educate patients and families for disease management.
• Coordinate transitions and discharge planning to reduce readmissions.
• Maintain timely documentation in the EMR.
• Supervise Home Health Aides as needed.
• Participate in on-call rotations as appropriate.
