Job summary

PruittHealth is seeking a compassionate Home Health RN Case Manager to deliver skilled nursing visits in patients' homes and coordinate care across the interdisciplinary team. You will assess, plan, implement, and evaluate patient care with autonomy and clinical support.

Expect flexible scheduling, ongoing training, and strong leadership. You will educate patients and families, supervise aides, and document care using mobile tools, including an AI-assisted iPad workflow.

Qualifications

• Active, unrestricted RN license.
• Valid driver's license and reliable transportation.
• CPR certification.
• 1+ year RN experience (home health or acute care preferred).

Responsibilities

• Provide skilled nursing visits in the patient’s home.
• Complete assessments, start-of-care visits, and care plans.
• Collaborate with the interdisciplinary care team.
• Supervise LPNs and Home Health Aides.
• Document care using an iPad with AI assist.
• Educate patients and families to promote independence.
• Ensure continuity, quality, and safety of patient care.
