Job summary

Mass General Brigham's VNA & Hospice of Cooley Dickinson, Inc. seeks an RN Case Manager to provide skilled care and case management for patients and families, in line with State Nurse Practice Act and professional standards.

Reports to the Clinical Team Manager and works onsite at 168 Industrial Drive. Requires nursing program graduate, MA RN license, CPR, 1+ year medical/surgical or home care/hospice experience; strong communication and organizational skills; valid driver's license.

Qualifications

• Graduate of an approved, accredited nursing program.
• Current MA RN license is required.
• CPR certification required.
• 1+ year Medical/Surgical nursing experience; home care/hospice preferred.
• Strong verbal and written communication and interpersonal skills.
• Good organizational and time management abilities; valid driver's license with reliable transportation.

Responsibilities
