Hartford HealthCare at Home is seeking a Registered Nurse (RN) to join our Hartford HealthCare at Home team in Connecticut. The role covers multiple towns including Union, Ashford, Eastford, Chaplin, Hampton and nearby areas, delivering patient-centered homecare and coordinating with interdisciplinary teams.

The ideal candidate holds a CT RN license, has at least 1 year of RN experience (homecare preferred), and is equipped with a valid driver's license and computer proficiency to document care

Qualifications

CT RN license required.
RN experience: 1 year; homecare experience preferred.
Valid driver’s license and reliable transportation required.
Computer proficiency required.

Responsabilités

Perform comprehensive assessments to establish an individualized care plan.
Facilitate communication between the patient and the interdisciplinary team.
Develop and maintain a therapeutic relationship by providing education and promoting self-advocacy and independence.
Utilize state-of-the-art technology to document skilled services provided.
