BJC Home Care in Sullivan, MO is seeking a part-time RN to provide skilled nursing in patients' homes. This ongoing staff role is 32 hours per week, with typical hours Monday through Friday 8:00 am to 4:30 pm and an on-call rotation; off days Wednesday or Thursday.
As a case manager or team member, you will assess patient needs, develop, implement and document plans of care, and coordinate with families and the interprofessional team to ensure continuity of care.

Specialty home care programs also are available, including adult and pediatric asthma, cardiac, diabetes, orthopedic and wound care programs.

Evaluates the client and furnishes services requiring substantial and specialized skill, appropriate preventive and rehabilitative nursing procedures, and instructions to assist the client in learning appropriate self-care techniques. When assigned as case manager, the staff nurse is responsible for coordinating all aspects of care related to that patient.
