Graham Healthcare Group is seeking a Registered Nurse (RN) in Home Health to provide skilled nursing care in the Ypsilanti, Michigan area. This ongoing staff position offers 40 hours per week with an 8-hour shift and requires driving to patient locations within an assigned territory.

Qualified candidates must hold an active RN license in the state of work and have at least one year of RN experience. A valid driver’s license and comfortable travel are required; a $10,000 first-year success bonus

Qualifications

Active Registered Nurse, RN license in the state you work.
One year of RN experience and the clinical expertise to deliver quality patient care.
Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required.

Responsibilities

Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you
Perform patient assessments, medication management, and care planning
Educate and empower patients and families on disease management and treatment plans so they feel confident, informed, and cared for
Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation
Work independently, but never alone
Travel to patient homes within assigned territory
