Job summary

MercyOne Home Care & Hospice – Mason City, part of Trinity Health, is seeking an experienced Home Care Registered Nurse (RN) to provide comprehensive in-home care Monday through Friday, with weekend rotation and on-call weekend shifts as scheduled.

You’ll assess patients, develop care plans with the care team, administer medications, educate families, and ensure documentation and regulatory compliance while delivering compassionate, patient-centered care in a home setting.

Qualifications

• Active RN license in Iowa.
• Strong clinical assessment and critical thinking skills.
• Ability to work independently and manage time effectively.

Responsibilities

• Perform comprehensive patient assessments and identify care needs.
• Develop and implement individualized care plans with interdisciplinary teams.
• Administer medications and treatments, monitor responses, and adjust care as needed.
• Educate patients and families on disease management, medications, and wellness.
• Monitor vital signs and manage chronic conditions with timely interventions.
• Document care activities and maintain regulatory compliance.
• Coordinate care with physicians, therapists, and other professionals.
• Provide emotional support to patients and families.
• Participate in ongoing education and quality initiatives.

Employment Type: Full time Shift: Description:

• Home Care Registered Nurse (RN) – MercyOne/Trinity Health Home Care, Mason City Monday - Friday 8am-4:30 pm; Weekend rotation based on how many nurses are employed. On call only Saturday and Sunday 8am-4:30 pm on your scheduled weekend. No weekday or weekend evening call.
• $10,000 Sign-On Bonus

What You’ll Do

• Administer medications and treatments, monitor patient responses, and adjust care as needed.
• Educate patients and families on disease management, medications, and wellness strategies.
• Monitor vital signs and manage chronic conditions with timely interventions.
• Accurately document all care activities and maintain compliance with healthcare regulations.
• Coordinate care with physicians, therapists, and other healthcare professionals.
• Provide emotional support and guidance to patients and their families.
• Participate in ongoing education and quality improvement initiatives.

What You’ll Need

• Active RN license in the state of Iowa.
• BSN preferred; ADN accepted.
• Previous experience in home health or community nursing is a plus.
• Strong clinical assessment and critical thinking skills.
• Ability to work independently and manage time effectively.
• Valid driver’s license and reliable transportation.
