Job summary

All Care in Macon, GA is seeking a part-time Home Health Registered Nurse to join our team serving individuals with developmental disabilities in their own homes. The role covers 20 to 30 hours per week and involves weekly assessments, supervisory visits, and coordination of medical care.

The ideal candidate holds an active GA RN license and has experience in home health settings, with strong documentation and patient communication skills.

Qualifications

Home Health RN

Part Time Management Macon, GA, US

Part-Time Home Health RN working with individuals with developmental disabilities.

20 to 30 hours per week.

DUTIES:

• Provide weekly assessments of individuals served
• Conduct supervisory visits and document results
• Annual oversight of health, risk and safety assessments using the Health Risk Screening Tool (HRST)
• Review of data tracking and documentation
• Staff training and oversight including proxy care
• Coordinate medical visits for clients
• Medication reconciliations
• Other duties as assigned
