Job summary

The Palms Home Care is expanding into the Gainesville market, seeking an experienced Home Health RN to join our growing team and lead field nursing efforts in this new territory.

The ideal candidate brings Medicare home health and OASIS experience and is comfortable working in a fast-paced, growth-oriented environment.

Qualifications

• RN license in Florida (unrestricted).
• Home health field experience strongly required.
• OASIS experience strongly required.
• Medicare-certified Home Health experience preferred.
• Kinnser/WellSky experience preferred.
• Experience with a home health startup/market expansion preferred.
• Strong assessment, documentation and care coordination skills.
• Ability to independently manage a field caseload and schedule.
• Current CPR certification.
• Valid driver's license and reliable transportation.

Responsibilities

• Provide skilled nursing care to patients in their homes throughout the Gainesville service area.
• Complete comprehensive patient assessments and develop individualized plans of care.
• Complete Start of Care (SOC), Resumption of Care (ROC), Recertification and Discharge visits.
• Complete accurate and timely OASIS assessments and clinical documentation.
• Perform medication management, wound care, disease management, patient education and other skilled nursing services.
• Coordinate patient care with physicians, therapists, LPNs and other members of the interdisciplinary team.
• Monitor patient conditions and communicate significant changes to physicians and clinical leadership.
• Educate patients and caregivers regarding medications, disease processes, treatment plans and self-care.
• Maintain timely and accurate documentation in accordance with Medicare, agency and regulatory requirements.
• Assist in supporting the growth and development of our newly established Gainesville service area.

Job description

The Palms Home Care is expanding into the Gainesville market beginning October 1st, and we are looking for an experienced Home Health Registered Nurse (RN) to join our growing team.

This is an exciting opportunity for an experienced field RN who enjoys autonomy and wants to be part of building and growing a new home health territory. The ideal candidate will have strong Medicare Home Health and OASIS experience and be comfortable working in a fast-paced, growth-oriented environment.

Responsibilities

• Provide skilled nursing care to patients in their homes throughout the Gainesville service area.

• OASIS experience strongly required.

• Kinnser/WellSky experience preferred.

• Previous experience with a home health startup, new branch, market expansion or growing census is highly preferred.

• Strong assessment, documentation and care coordination skills.

• Ability to independently manage a field caseload and schedule.

• Current CPR certification.

• Valid driver's license, reliable transportation and automobile insurance.

Compensation & Benefits

Salary: $80,000-$90,000 per year, based on experience
