Job summary

Medix is seeking a Home Health RN in Evesham, New Jersey to assess, plan and evaluate patient care in the home setting. The RN will coordinate care with the healthcare team and families, supervise Home Health Aides, and promote high-quality, cost-effective services.

The role requires an active New Jersey RN license and 3–4 years of nursing experience; duties include visiting 5–6 patients per day across Burlington, Camden and Gloucester Counties. Mileage is reimbursed at government rates.

Qualifications

• Active New Jersey RN license.
• 3–4 years of nursing experience preferred in Med-Surg, ICU/CCU, Home Health.
• OASIS experience — required for candidates from certified home health; Home Health experience preferred.

Responsibilities

Home Health RN

• Schedule:Monday–Friday, 8:00 AM–5:00 PM
• Pay Rate:$45.00–$50.00/hourPay Scale Based on Experience
• Mileage:Reimbursed at the government rate

Position Summary

The Home Health RN is responsible for assessing, diagnosing, planning, implementing, and evaluating patient care in the home using evidence-based practice and outcome identification.

The RN will coordinate patient care and collaborate extensively with healthcare team members, patients, families, and significant others to develop and implement individualized plans of care.

The RN will also supervise Home Health Aides, promote quality care, and support efficient utilization of agency resources.

The RN will visit approximately5–6 patients per day throughout Burlington, Camden, and Gloucester Counties.

Required Qualifications

• ActiveNew Jersey RN license

3–4 years of nursing experiencepreferred in:

• Med-Surg
• ICU/ICE
• CCU
• Home Health

Ability to perform and document manual vital signs

Preferred Qualifications

• OASIS experience —required for candidates coming from certified home health
• Certified Home Health experience
