Job summary

Royale Care VA is seeking an upbeat Home Health Registered Nurse (RN) licensed in Virginia to provide in-home assessments, develop plans of care, and collaborate with the Clinical Manager to deliver high-quality care.

The ideal candidate has at least one year of health care/home health experience, some supervisory exposure, and the ability to exercise independent clinical judgment while coordinating patient referrals and services.

Qualifications

• Must be registered to practice in Virginia (VA) or Compact State License.
• At least 1 year of experience in health care/home care.
• At least 1 year of supervisory experience (preferred).
• Knowledge of home health principles and practices.

Responsibilities

• Perform in-home patient assessments and develop care plans.
• Complete weekly patient home visits.
• Meet minimum required number of visits per month.
• Coordinate patient referrals and patient care.
• Ensure ongoing assessment of patients' needs.
• Develop, implement, and update individualized care plans.

Job description

We are looking for an upbeat, goal oriented person to fill the position of Home Health Registered Nurse (RN).

The Registered Nurse (RN) who has graduated from an accredited school of nursing and is currently licensed to practice in the state of Virginia . She/he completes initial assessments for home bound patients in their place of residence in accordance with attending physician orders, develops plans of care in collaboration with the Clinical Manager and strives to provide the highest quality of care. In addition, the RN collaborates in the provision of other therapeutic services are under the supervision and direction of the Director of Clinical Services/Clinical Manager to develop a comprehensive care plan.

The ideal candidate must demonstrate the ability to exercise independent clinical judgement, have experience in Home Health OASIS completion and be a good collaborator.

Responsibilities will include but are not limited to:

• Performing in-home patient assessments and developing care plans
• Completing weekly patient home visits
• Complete minimum required number of visits per month
• Coordinating patient referrals and patient care
• Assuring continued assessment of patients needs
• Assuring the development, implementation, and updates of the individualized plan of care

QUALIFICATIONS

1. Must be registered to practice in the state of Virginia (VA) or Compact State License.

2. Must have at least one year of experience in health care/home care.

3. At least one year experience in a supervisory position (preferred).

4. Knowledge and ability to apply home health principles and practices.
