Aveanna Healthcare in Des Moines is seeking a Registered Nurse (Home Health) for part-time visits in the Des Moines service area. You will deliver one-on-one home-based nursing care, conduct in-home assessments, and develop patient-centered care plans to promote recovery and safety.

Qualifications

• Holds an active Registered Nurse license in the state of practice.
• Minimum of one year of clinical nursing experience.
• Valid driver's license, reliable transportation, and auto insurance.
• CPR certification (or ability to obtain).
• Ability to perform in-home nursing assessments and plans.
• Commitment to patient-centered, home-based care.
• Strong organization and documentation skills.

Responsibilities

• Serve as the primary clinical partner for patients and families, delivering one-on-one nursing care in the home that promotes recovery, safety, and independence.
• Conduct in-home assessments to evaluate physical, emotional, and environmental needs and translate findings into patient-centered care plans.
• Provide skilled nursing interventions—including wound care, infusions, catheter care, medication management, post‑operative care, and chronic disease management.
• Educate and empower patients and caregivers through clear instruction, coaching, and support for home care at home.
• Monitor patient progress closely, identify changes in condition, and communicate with physicians and the care team to adjust plans.
• Coordinate care across providers to ensure continuity and high-quality outcomes throughout the home health journey.
• Complete timely, accurate documentation reflecting the clinical picture and outcomes.

Monday - Friday Part-Time visits for the Des Moines branch service area. Full-time, daytime hours, Monday - Friday + participation in team shared on-call rotation.

The Nurse We’re Seeking

• Holds an active Registered Nurse (RN) license in the state of practice with at least one (1) year of clinical nursing experience.
• Is patient‑focused, organized, and motivated by delivering high-quality care.
• Has a current driver’s license, reliable transportation, and auto insurance.
