Responsibilities

• Develop and initiate individualized care plans with patients and caregivers
• Deliver physician-ordered nursing services and preventive/rehabilitative procedures
• Educate patients and caregivers to promote health and independence
• Identify and implement discharge planning needs
• Coordinate necessary equipment and services
• Supervise LPNs, LVNs, and HHAs
