Job summary

Aveanna Healthcare is seeking a Registered Nurse to provide in‑home health care in Sussex County. As part of our Home Health team, you will deliver patient‑centered nursing care, perform in‑home assessments, and coordinate with physicians and caregivers to support recovery and independence.

You’ll educate patients and families, manage wounds and medications, document care plans accurately, and ensure safe, high‑quality outcomes.

Qualifications

• Active RN license in state of practice with at least 1 year of clinical nursing experience
• Patient‑focused, organized, and motivated by delivering high‑quality care
• Current driver’s license, reliable transportation, and auto insurance
• CPR certification (or ability to obtain)

Responsibilities

• Deliver one‑on‑one nursing care in the home that promotes recovery, safety, and independence
• Conduct in‑home assessments and translate findings into patient‑centered care plans
• Provide skilled nursing interventions including wound care, infusions, catheter care, medications, postoperative care, and chronic disease management
• Educate and empower patients and caregivers to manage symptoms and daily care at home
• Monitor progress, communicate with physicians to adjust plans, and prevent hospitalizations
• Coordinate care across providers to ensure continuity and quality outcomes
• Complete timely, accurate documentation reflecting the clinical picture

What You’ll Do

• Serve as the primary clinical partner for patients and families, delivering individualized, one‑on‑one nursing care in the home that promotes recovery, safety, and independence.

• Coordinate care across providers and services, ensuring continuity, clarity, and high‑quality outcomes throughout the patient’s home health journey.
• Complete timely, accurate documentation that reflects the full clinical picture and supports quality, compliance, and measurable outcomes.

The Nurse We’re Seeking

• Holds an active Registered Nurse (RN) license in the state of practice with at least one (1) year of clinical nursing experience
