Job summary

Aveanna Home Health in Beaufort County is seeking a dedicated Registered Nurse (RN) to provide in-home, one-on-one nursing care on a full-time basis. You will conduct in-home assessments, develop care plans, and deliver skilled nursing interventions tailored to each patient’s goals and needs.

Requirements include an active RN license in the state, at least 1 year of clinical nursing experience, a valid driver’s license, and CPR certification.

• CPR certification (or ability to obtain).

Responsibilities

• Deliver one-on-one in-home nursing care to patients.
• Conduct comprehensive in-home assessments and translate findings into care plans.
• Provide skilled nursing interventions including wound care, infusions, catheter care, medication management, post-operative and chronic disease care.
• Educate and empower patients and caregivers with clear instructions and support.
• Monitor patient progress and communicate with physicians and care team to adjust plans.
• Coordinate care across providers to ensure continuity and high-quality outcomes; complete timely documentation.

• Monitor patient progress closely, identify changes in condition early, and communicate effectively with physicians and the interdisciplinary care team to adjust plans and prevent avoidable hospitalizations.
• Coordinate care across providers and services, ensuring continuity, clarity, and high‑quality outcomes throughout the patient’s home health journey.
• Complete timely, accurate documentation that reflects the full clinical picture and supports quality, compliance, and measurable outcomes.

The Nurse We’re Seeking
