Job summary

Mercy Home Health, an agency of Trinity Health At Home, seeks an experienced Home Care RN to deliver primary nursing care to patients in their homes, following physician orders and organizational policies.

You will conduct initial assessments, develop care plans with the Population Health Case Manager, and support Home Healthcare Aides and LPNs, ensuring a patient-centered approach and flexibility. This role offers a $10,000 signing bonus and comprehensive benefits.

Qualifications

• Graduate of an approved nursing education program.
• Licensure as a Registered Nurse in the state of PA.
• One (1) year experience as a professional care nurse.
• Must have current Driver's license and reliable transportation.

Responsibilities

• Provide primary nursing care to home-based patients as ordered by the physician and in accordance with organization policies.
• Perform initial assessment and care plan development in collaboration with the Population Health Case Manager.
• Provide functional support/supervision to Home Healthcare Aides and LPNs as needed.
• Deliver therapeutic interventions, overall care management and contribute to high patient satisfaction.

Home Care RN position summary

Provides primary nursing care to home based care patients as ordered by the physician, and in accordance to the organization’s policies and procedures. The RN is responsible for the initial assessment and care plan development in collaboration with the Population Health Case Manager. Provides functional support/supervision to Home Healthcare Aides and LPNs as needed. Provides therapeutic intervention, overall care management and achievement of top decile client outcomes and patient satisfaction.
