Home Care Providers of Texas Inc. is seeking compassionate PRN Registered Nurses (RNs) to cover the North Fort Worth, TX area. You will leverage AI-driven Roger to reduce charting time while delivering high-quality home health care.

Formación

• Active RN license valid in TX or Compact license.
• Current CPR certification required.
• Experience delivering home health nursing, geriatric focus preferred.
• Must be self-driven with strong organizational skills.
• Demonstrates compassionate, patient-centered care.
• Valid driver’s license and reliable vehicle required.

Responsabilidades

• Deliver skilled nursing care in patient homes per physician orders and care plan.
• Collaborate with care team to develop and update individualized plans of care.
• Administer nursing treatments and manage clinical activities.
• Monitor patient progress and communicate updates to care team, patient, and family.
• Additional duties as assigned.

Essential Job Functions

• Deliver Skilled Nursing Care - Provide high-quality, comprehensive nursing care in the patient's home in accordance with the physician’s orders and individualized plan of care.
• Collaborate on Plan of Care - Partner with the interdisciplinary care team to develop, implement, and update personalized care plans that address each patient’s unique needs.
• Manage Treatments and Interventions - Administer nursing treatments and manage clinical activities to support optimal health outcomes for each patient.
• Monitor and Evaluate Progress - Continuously assess patient progress and the effectiveness of services; communicate findings with the care team, patient, family, physician, and case manager.
• Additional duties as needed.

Requirements

• Active RN License (TX/Compact)
• Active CPR Certificate
• Home Health Experience – Prior experience ingeriatric home health care.
• Self-Starter with Organizational Skill
• Compassionate Care Approach
• Valid Driver’s License
