Job summary

Elara Caring is seeking a Registered Nurse Home Health for the Rockford, IL area on a PRN basis. Prior home health experience is preferred. You will assess patients, perform initial and ongoing assessments, and provide nursing care per plan of care while protecting HIPAA privacy.

Qualifications

• Current RN license in-state.
• Graduate of an accredited nursing program (certificate, diploma, ADN, or BSN).
• 1+ year of nursing experience in a clinical setting.
• Home health experience preferred but not required.
• Ability to lift up to 50 pounds and travel to patient homes.
• Valid driver's license or reliable transportation.

We are looking for an experienced Home Health RN to support the Rockford, IL area and surrounding communities on a PRN basis. Prior home health experience is required for this position.

What is Required?

• Current State license as a Registered Nurse.
• Graduate of an accredited Certificate, Diploma, Associates or Baccalaureate School of Nursing.
• 1+ year of experience as a nurse in a clinical care setting.
• Home Health experience is preferred, but not required.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.

Pay & Benefit Information

The base salary for this position is $55 dollars per point based on the company's good faith estimate at the time of posting. Actual pay will be determined based on factors such as education, experience, skills, and internal equity.
