What You’ll Do:

• Conduct start‑of‑care comprehensive assessments and complete OASIS in a timely manner.
• Draft care plans, reconcile patient medications, and ensure orders for care are accurate and complete.
• Prepare accurate, orderly, and timely documentation.
• Coordinate care with patient and provide proper reports to Case Manager.
• Provide health care instructions to the patient/family as appropriate per assessment and plan.

• An active RN license in the state in which you practice or New Grad with eligibility to take NCLEX.
