Park Nicollet is recruiting a Registered Nurse to join the Home Care team. The RN assesses patient care needs, provides skilled nursing and education to patients and families in home settings, and coordinates medical, social, and financial resources for a comprehensive plan of care.

Schedule requires 40 hours per month with 6-7 week weekend duties; some weekend/on-call may occur. Benefits packages are offered for eligible positions.

Qualificações

• Two years RN experience or more.

Responsabilidades

• Assess and evaluate patient care needs in home/community.
• Provide skilled nursing care and teaching to patients and families.
• Coordinate resources to develop a comprehensive plan of care.
