Job summary

UnitedHealth Group's Mederi Caretenders Home Health division seeks a dedicated Registered Nurse to deliver and coordinate in-home nursing care, ensuring compliance with physician orders and agency policies.

You'll collaborate with physicians, home health aides, and care teams to develop comprehensive plans of care, perform assessments, and support patients and families across diverse home environments.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Valid driver’s license, vehicle insurance, and access to dependable transportation.
• Current CPR Certification or ability to complete within 90 days of hire.
• Ability to function in any home situation regardless of client demographics.

Responsibilities

• Initiates, develops, implements, and revises the plan of care in collaboration with the physician and other health care professionals
• Supervises care provided by home health aides and licensed nurses, provides instruction, and assigns tasks according to regulations
• Provides required supervisory visits
• Completes comprehensive assessments (OASIS) including medication reconciliation accurately
• Documents patient visits per policy and payer requirements
• Communicates information timely with agency staff and physicians to ensure quality care
• Participates in on-call and weekend rotations as needed to meet patient needs
• Adheres to the agency's utilization management model
