Job summary

Pathwellhealth in Connecticut seeks a Registered Nurse to assess patient health problems, develop and implement nursing care plans, and maintain medical records during home health visits.

The RN will coordinate with physicians and other care team members, educate clients and families on home health procedures, and modify plans as patients respond to treatment. Requires CT RN license and CPR; home health experience preferred; PRN available.

Qualifications

• Associates degree required; Bachelor’s Degree from school of nursing preferred
• Current Connecticut RN license (state licensure)
• Current CPR certification (required)
• Home health care experience preferred
• Bilingual (Spanish) preferred
• Driver’s license and reliable transportation
• Strong interpersonal and communication skills
• Computer literate and able to document care

Responsibilities

• Assess patient health problems and needs; develop and implement nursing care plans
• Maintain medical records and document care accurately
• Provide skilled nursing treatments as ordered by physician
• Complete evaluation tasks including vital signs and medication reviews
• Coordinate with physicians, therapists, and other care team members
• Assess and chart patient condition at each visit
• Monitor and report symptoms or changes in patients’ conditions
• Provide education to patients and families on home health care procedures
