Job summary

CenterWell Home Health is seeking a Home Health Registered Nurse to provide one-on-one care in patients' homes, under the supervision of the Clinical Manager. You will develop and adjust care plans with physicians, monitor patient progress, and educate families on disease management and treatments.

You will work with a multidisciplinary team to ensure comprehensive, quality care in a home setting, including supervising Home Health Aides and coordinating documentation.

Qualifications

• Active, unrestricted RN license in the United States
• Current CPR certification
• Experience in med-surg, ICU, ER, or acute care
• Reliable transportation and a valid driver’s license
• Ability to work independently in patients’ homes

Responsibilities

• Provide skilled nursing care to patients in their homes, including meds administration and wound care.
• Develop and implement individualized care plans with physicians and clinicians.
• Monitor patients' conditions and report changes to the care team.
• Educate patients and families on disease management and medications.
• Maintain accurate nursing records and coordinate with healthcare professionals.
• Report patient progress to physician and Clinical Manager.
• Oversee Home Health Aides and ensure proper documentation.

As a CenterWell Home Health nurse, reporting to the Clinical Manager, you will provide personalized, one-on-one care that helps patients regain independence in the comfort of their homes. Working with a dedicated team of physicians and clinicians, you'll manage care plans that support recovery and help patients get back to the life they love.

As a Home Health Registered Nurse , you will:

• Provide skilled nursing care to patients, working onsite in their homes, including administering medications, performing wound care, and conducting patient assessments.
• Develop and implement individualized care plans in collaboration with physicians and other healthcare professionals.
• Monitor patients' conditions and report changes.
• Educate patients and their families on disease management, medication, and treatment options.
• Maintain accurate records of patient care and coordinate with other healthcare professionals.
• Report patient care and condition progress to patient's physician and Clinical Manager.
• Oversee the work of Home Health Aides and monitor their appropriate completion of documentation.

Required Qualifications:

• Med surg, ICU, ER, or acute experience
• Current and unrestricted Registered Nurse licensure
• Current CPR certification
• Experience collaborating with a team of healthcare professionals
• Valid driver's license, auto insurance and reliable transportation

Preferred Qualifications:

• One year nursing experience
• Home Health experience

Pay Per Visit/Unit Rate

$49.00 - $69.00

Additional Information

TB Statement: This role is considered patient facing and is part of Humana's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.

Driving Statement: This role is part of Humana's driver safety program and therefore requires an individual to have a valid state driver's license and are expected to maintain personal vehicle liability insurance. Individual must carry vehicle insurance in accordance with their residing state minimum required limits, or $25,000 bodily injury per person/$25,000 bodily injury per event /$10,000 for property damage or whichever is higher.
