Job summary

Yale New Haven Health seeks a Home Care Nurse to provide skilled nursing care to clients per physician orders and the agency care plan. You will admit clients, develop care plans, implement and evaluate them, and coordinate services with families and other members of the multidisciplinary team.

This role requires strong clinical knowledge, excellent verbal and written communication, independence, and the ability to assess patient needs and adjust plans.

Qualifications

• Nursing diploma or ASN; BSN preferred.
• Two years nursing experience preferred; home care experience desirable but not required.
• Registered Nurse with Connecticut license (or eligible).

Responsibilities

• Admits clients to service; develops care plan; implements, evaluates, and revises plan of care as indicated.
• Provides professional nursing care including preventative, therapeutic, and rehabilitative nursing per physician orders and the care plan.
• Implements or delegates 24 hour care plan and home health aide services.
• Coordinates services with the client, family and others involved in the care plan
• Evaluates client and notifies physician of changes; revises care plan as needed
• Identifies client and family needs for other health or community services and refers when appropriate
• Promotes safe environment for the client
• Delegates to home health aide; develops written plan of care and supervision
• Arranges supervision of home health aide when necessary
• Documents all services in the clinical record
• Completes required statistical and financial information
• Participates in at least 12 hours of education per year
• Participates in other assignments to implement agency programs
• Maintains productivity level as directed by Supervisor of Clinical Services
• Projects positive attitude about agency mission and policies

• 10. Documents all services and coordination in the clinical record.
• 11. Completes all required statistical and financial information.
• 12. Assumes responsibility to participate in at least 12 hours of education per year.
• 13. Participates in other assignments as necessary to implement agency programs.
• 14. Maintains the productivity level as established by the Supervisor of Clinical Services.
• 15. Projects a positive attitude and enthusiasm about agency mission, philosophy, and policies.

Qualifications

EDUCATION

Nursing Diploma or Associates Degree in Nursing. Bachelors in Nursing preferred.

Experience

Two years nursing experience preferred. Previous home care experience desirable, but not required.

LICENSURE

Registered Nurse with Valid (or eligible) Connecticut license.

Special Skills

Excellent clinical knowledge, verbal/written skills, and organizational skills. Must be able to work independently. Able to competently assess patients' needs and follow through accordingly. Strong ability to work with providers and members of the multidisciplinary team.

PHYSICAL DEMAND

Ability to perform tasks involving physical activity which may include heavy lifting and extensive bending and standing.
