Job summary

AccentCare seeks a compassionate Registered Nurse to provide weekend home health visits to adult and geriatric patients. In this role, you will complete in‑home assessments, develop and update care plans, administer medications and treatments, and provide wound care and IV therapy as needed.

Qualifications

• Licensed Registered Nurse (RN) with home health competencies.
• Ability to educate patients and families on disease management and safety.
• Proficiency documenting care in an electronic health record (EHR).

Responsibilities

• Perform in‑home assessments for adults and geriatric patients.
• Develop and update individualized care plans.
• Administer medications and treatments as prescribed.
• Provide wound care and IV therapy as needed.
