They will manage a caseload, coordinate with a multidisciplinary team, and communicate effectively in line with agency policies. • Physical ability to lift, transfer, and maneuver patients and objects weighing at least 50 pounds.
• Competent in assessing patient conditions, developing care plans, and educating patients.
• Must possess sensory acuity for visual and auditory assessments.
• Awareness of hazards such as exposure to bloodborne pathogens, infectious diseases, and safety risks during home visits.
• Valid driver's license and operational vehicle required.
