Job summary

CenterWell Home Health is seeking a licensed Registered Nurse to provide skilled, one-on-one home health care. You will administer medications, perform wound care, and conduct patient assessments under the supervision of the Clinical Manager.

Collaborate with physicians to develop care plans, monitor progress, educate families, and maintain accurate documentation. This role requires driving to patient homes and supervising Home Health Aides, with a 40-hour work week.

Qualifications

• Must have RN licensure in good standing.
• Current CPR certification is required.
• At least 1 year of nursing experience.
• Experience collaborating with a healthcare team is essential.
• Valid driver's license and reliable transportation are required.

Responsibilities

• Provide skilled nursing care in patients' homes, including meds administration and wound care.
• Develop and implement individualized care plans with physicians and other professionals.
• Monitor patient conditions and report changes to the physician and Clinical Manager.
• Educate patients and families on disease management and treatment options.
• Maintain accurate patient records and coordinate with other professionals.
• Oversee Home Health Aides and ensure proper documentation.

As a CenterWell Home Health nurse, reporting to the Clinical Manager, you will provide personalized, one-on-one care that helps patients regain independence in the comfort of their homes. Working with a dedicated team of physicians and clinicians, you'll manage care plans that support recovery and help patients get back to the life they love.

As a Home Health Registered Nurse , you will:

• Provide skilled nursing care to patients, working onsite in their homes, including administering medications, performing wound care, and conducting patient assessments.
• Develop and implement individualized care plans in collaboration with physicians and other healthcare professionals.
• Monitor patients' conditions and report changes.
• Educate patients and their families on disease management, medication, and treatment options.
• Maintain accurate records of patient care and coordinate with other healthcare professionals.
• Report patient care and condition progress to patient's physician and Clinical Manager.
• Oversee the work of Home Health Aides and monitor their appropriate completion of documentation.

• Med surg, ICU, ER, or acute experience
• Current and unrestricted Registered Nurse licensure
• Current CPR certification
• Experience collaborating with a team of healthcare professionals
• Valid driver's license, auto insurance and reliable transportation

• Home Health experience

TB Statement:

This role is considered patient facing and is part of Humana's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.

Driving Statement:

This role is part of Humana's driver safety program and therefore requires an individual to have a valid state driver's license and are expected to maintain personal vehicle liability insurance. Individual must carry vehicle insurance in accordance with their residing state minimum required limits, or $25,000 bodily injury per person/$25,000 bodily injury per event /$10,000 for property damage or whichever is higher.

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.
