Job summary

Proforma Health Inc is seeking a Registered Nurse for low-acuity in-home daytime care in Denton, TX. You will provide direct nursing services in patients' homes, delivering compassionate, patient-centered care.

You will assess, administer medications, perform wound care, and monitor vital signs, while documenting in the EHR and collaborating with families and the care team to promote safety, independence, and comfort.

Qualifications

• RN licensed to practice in Texas.
• Experience in home health care preferred.
• Strong clinical skills in assessments, meds and wound care.
• Excellent communication and teamwork abilities.
