Job summary

American River Home Care is seeking an experienced Registered Nurse (RN) in California to join our team delivering high-acuity, in-home care through our Advance Care at Home program. Open shifts vary between 8-hour shifts, Monday through Friday with rotating weekends.

You will provide routine and PRN nursing visits, lead care plans, and work 1:1 in patients' homes to reduce hospital readmissions while maintaining hospital-level attention.

Qualifications

• Active RN license in California.
• Med-Surg experience 1–2 years preferred.
• Home health or hospice experience is a plus.
• Ability to work independently in the field.
• Strong communication, adaptability, and problem-solving skills.
• Valid driver’s license and reliable transportation.

Responsibilities

• Provide routine and PRN nursing visits—like a med-surg floor, but in a home setting.
• Deliver hands-on care (vital signs, wound care, IVs, meds, monitoring).
• Serve as the clinical lead in managing patient care plans and communicating changes.
• Work shift-style schedules, including every other weekend.
• Use clinical judgment daily—this is not intermittent home health.
• Build relationships with patients and families to improve outcomes.

What You'll Do:

• Build deep relationships with patients and families while improving outcomes.

What We're Looking For:

• Active RN license in California
• Med-Surg experience is preferred (1–2+ years)
• Home health or hospice experience is a plus, but not required
• Must be comfortable working independently in the field
• Strong communication, adaptability, and problem-solving skills
• Valid driver’s license, reliable transportation, and auto insurance
• Willingness to cover PRN needs and work every other weekend
