Job summary

CenterWell Home Health, a Humana subsidiary, is seeking a Home Health Physical Therapist Assistant to help patients regain strength and independence in the comfort of their homes, under the guidance of a Physical Therapist. You will work with a caring team to deliver personalized care and coordinate with the office to maintain documentation.

The role requires a current PT license, a valid driver’s license, CPR certification, and English fluency.

Qualifications

• Must hold current state license to practice Physical Therapy.
• Valid driver's license and reliable transportation.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.
• Comprehensive knowledge of geriatric population changes.
• Read, write, and speak fluent English.

Responsibilities

• Administer the physical therapy treatment plan to patients, working onsite in patients' homes, as developed by the Physical Therapist.
• Administer non-complex active and passive manual therapeutic exercises, therapeutic massage, traction heat, light, cold, water, and electrical modalities to patients.
• Document client records and submit clinical documentation to the office on time each week.
• Attend educational meetings and coordinate department in-services.
• Notify Physical Therapist or Branch Director of any equipment failures, lack of supplies, or staff or client changes.
• Apply concepts of infection control and universal precautions in performing patient care activities.
