CenterWell Home Health, a Humana company, is hiring a Home Health Physical Therapist Assistant to deliver in-home PT services under a PT plan and collaborate with a care team. The role requires a state PT license, CPR, and a valid driver's license, with a 40-hour work week and a $5K sign-on bonus.

Salary ranges from $64,000 to $87,500 annually, with comprehensive benefits and opportunities to impact geriatric recovery in patients' homes.

Qualifications

• Current state license to practice Physical Therapy.
• Valid driver's license, auto insurance and reliable transportation.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.
• Knowledge of geriatric physical, emotional, social and biological changes.
• Fluent reading, writing and speaking English.
• One year of PTA experience (preferred).

Responsibilities

• Administer the physical therapy treatment plan to patients, working onsite in their homes, as developed by the Physical Therapist.
• Perform non-complex active and passive manual therapy, exercises, modalities as prescribed.
• Document client records and submit clinical documentation to the office weekly.
• Attend educational meetings and coordinate department in-services.
• Notify Physical Therapist or Branch Director of equipment or staff changes.
• Apply infection control and universal precautions in patient care.
