• Current Colorado PTA license in good standing.
• Graduate of an accredited Physical Therapist Assistant program.
• Valid driver's license, reliable transportation, and auto insurance.
• Strong communication and documentation skills.
• Ability to work independently and adapt professionally in home-based setting.

Responsibilities

• Provide skilled therapy services in patient homes.
• Carry out established treatment plans involving therapeutic exercise, gait training, balance activities, and functional mobility intervention.
• Monitor and communicate patient progress and changes to supervising PT.
• Encourage patient participation and reinforce goals.
• Complete timely, accurate documentation in EMR.
• Collaborate with therapists, nurses, caregivers, and other care team members.
