Job summary

Infinity Hospice Care LLC is seeking a Physical Therapist Assistant (PTA) to implement treatment plans created by the Physical Therapist in patients' homes. You will coordinate changes with the PT and care team, educate patients and families on therapy plans and home exercises, and document progress for ongoing care and compliance.

Requirements include an Associate degree from an APTA-accredited program, current PTA license in good standing, CPR, and a valid driver's license.

Qualifications

• PTA applies a treatment program designed by the Physical Therapist.
• Communicates patient progress and changes to the PT and team.
• Maintain professionalism per ethical standards and board requirements.

Responsibilities

• Treat patients per a program designed by the Physical Therapist and coordinate with team.
• Educate patients and families about therapy plans, home exercise, and safety.
• Document patient treatment and progress for the PT and Agency.
• Participate in patient conferences and mandatory case reviews.
• Maintain timely documentation to comply with company policy.
