Job summary

UnitedHealth Group is seeking a Physical Therapist for home health in West Virginia. You will assess patient needs, determine and implement a treatment plan, and work to improve mobility, endurance, and overall well-being.

Coverage includes Greenbrier, Pocahontas and Monroe Counties, with a focus on safe, quality care in the home environment. The role requires current PT licensure in the state, CPR, and reliable transportation.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current CPR certification.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.

Responsibilities

• Evaluates patient functional status and PT needs and assists physician in therapy plan.
• Provides services within state practice guidelines and plan of care.
• Observes and reports patient response to treatment to supervising nurse/physician.
• Treats patients to relieve pain, restore function, and maintain maximal functioning.
• Instructs patients, families, and care teams about plan of care and progress toward goals.
• Visits patients per Plan of Treatment and submits progress notes per policy.
• Completes PT evaluations and develops plan of care within state guidelines.
• Reports outcomes of evaluation, goals, and anticipated frequencies of care.
