Job summary

Blossom Ridge Home Health in Sacramento, California is hiring a Full-Time Home Health Physical Therapist to assess and treat patients in their homes, develop plans of care, and collaborate with the care team to improve mobility, endurance and functional independence.

Responsibilities include evaluating patients, implementing treatment plans, documenting progress, and communicating with physicians and families.

Qualifications

• Licensed physical therapist in California.
• Valid driver’s license with reliable vehicle or access to transportation.
• CPR certification or ability to obtain within 90 days of hire.

Responsibilities

• Evaluates patient’s functional status and PT needs and assists in development of therapy plan of care.
• Provides PT services within scope of practice and in coordination with health care team.
• Observes and reports patient responses to treatment and changes in condition to supervising nurse/physician.
• Treats patients to relieve pain, restore function, and maintain maximum functioning.
• Instructs patient and family/caregivers regarding plan of care, limitations, and progress toward goals.
• Completes all patient evaluations and develops PT plan of care within state guidelines.
• Reports outcomes of evaluation, goals, and frequency of care.
