Luxe Home Health LLC is seeking a Physical Therapist (PT) or Physical Therapist Assistant (PTA) to provide exceptional home-based care in Oklahoma City, Norman, and surrounding areas.

We offer full-time and per diem opportunities with competitive per-visit pay, flexible scheduling, supportive leadership, and a collaborative team dedicated to improving patients' quality of life at home.

Competenze

• Valid Oklahoma PT or PTA license.
• CPR certification required.
• Reliable transportation and valid Oklahoma driver's license.
• Strong communication and documentation skills.
• Home health experience preferred.

Mansioni

• Evaluate patients' mobility, strength, and functional abilities.
• Develop and implement individualized treatment plans with physicians.
• Provide skilled therapy interventions and adjust plans as needed; monitor progress.
• Educate patients and caregivers on home exercise programs.
• Complete accurate and timely documentation.
