You will evaluate patients, build individualized plans of care, provide therapy in the home, supervise PTAs, document progress, and manage your own daily schedule and route across the OKC area and surrounding communities.

Qualifications

• Active PT license in Oklahoma is required.
• Home health experience is a plus but not required.
• Ability to work independently and manage own schedule.

Responsibilities

• Evaluate patients and build individualized plans of care based on their goals.
• Provide physical therapy treatment in the home, where progress translates to daily life.
• Supervise PTAs and adjust the plan of care as patients progress.
• Document evaluations and progress, and communicate with the physician and care team.
• Manage your own daily schedule and route across your caseload.
