Job summary

PARS Therapy in Mansfield, TX is seeking a compassionate Physical Therapist to join our dedicated home health team. You will assess patients' mobility, design individualized treatment plans, and deliver interventions at patients' homes to restore function and reduce pain.

Applicants must hold an active Texas PT license and a PT degree; home health experience is preferred. We offer comprehensive medical plans, PTO, a 401(k), and a supportive, patient-centered work environment.

Qualifications

• Active Texas Physical Therapist license required.
• Graduate degree in Physical Therapy from an accredited program.
• Experience in home health or rehabilitative settings preferred.
• Ability to adapt care plans to individual patient needs.

Responsibilities

• Evaluate patients’ physical abilities and functional limitations in a home health setting.
• Develop and implement individualized treatment plans to restore function and reduce pain.
• Provide therapeutic interventions including exercise, gait training, manual therapy, and balance retraining.
• Educate patients and caregivers on home exercise programs and safety techniques.
• Monitor progress and adjust plans to achieve outcomes.
• Document evaluations, treatment sessions, and progress notes in digital systems.
• Collaborate with physicians, nurses, and other therapists.
• Conduct home safety assessments and recommend fall-prevention strategies.
• Deliver compassionate care while empowering patients to stay independent.
