Job summary

National Church Residences Home Health is seeking a Full Time Physical Therapist to deliver one-on-one therapy in patients’ homes across targeted Ohio counties. You’ll build personalized plans, assess mobility, and guide patients through exercises while coordinating with a multidisciplinary team.

The role emphasizes clinical independence, meaningful patient relationships, and the ability to work remotely in the field to support seniors in maintaining independence at home.

Qualifications

• Doctorate, master's, or bachelor's degree in physical therapy from a CAPTE-accredited program.
• Current Ohio physical therapy license.
• 1 year experience in licensed field.
• Valid driver’s license, reliable transportation, and auto insurance; MVR standards.
• Strong communication and interpersonal skills, with the ability to speak, read, write, and understand English.
• Working knowledge of Windows, Microsoft Office, web-based tools, and electronic health records.

Responsibilities

• Evaluate, plan, provide, and direct skilled physical therapy for home-based patients.
• Turn home visits into one-on-one therapy focused on safety, strength, and mobility.
• Create individualized treatment plans around patient routines and goals.
• Coach patients and caregivers with practical tools to sustain progress.
• Collaborate with nursing and therapy teams to coordinate care and maintain documentation.
