A List Therapy Group seeks a Home Health Physical Therapist licensed in California to join as an independent contractor. Provide in-home physical therapy services, evaluate patients, and develop care plans within physician orders and payer requirements.

Qualifications

• Current California Physical Therapist license.
• Reliable transportation and valid driver’s license.

Responsibilities

• Provide physical therapy services in accordance with applicable law, physician orders, payer requirements, and the approved plan of care.
• Evaluate patients in their homes and provide skilled PT interventions to improve mobility, safety, strength, balance, independence, and quality of life.
• Develop, update, and implement care plans consistent with clinical judgment and payer requirements.
• Assess home environments and recommend equipment, modifications, safety strategies, or caregiver education.
• Document patient progress accurately and efficiently per payer, agency, and regulatory requirements.
