Job summary

Trilogy Home Healthcare Fort Myers is seeking a dedicated Physical Therapist to provide one-on-one services in a home health setting across Naples and North Naples territories. This full-time role also offers PRN/per diem opportunities and a patient-centered environment.

You will help patients improve mobility, strength and balance, set care goals, and ensure safety in the home. Florida licensure and at least 1 year of experience, preferably in home health, are required.

Qualifications

• Graduate of an accredited PT program.
• Current Florida PT license.
• Valid Florida driver's license, active vehicle insurance, and clear MVR.
• 1+ year of experience; Home Health experience preferred.

Responsibilities

• Provide one-on-one physical therapy services in a home health setting.
• Help patients improve mobility, strength, and balance.
• Set and manage care goals that promote recovery and independence.
• Evaluate the home environment for safety and mobility needs.
