Job summary
BAYADA Home Health Care is seeking a Home Health Physical Therapist to provide in-home care for adult and geriatric clients in Brockton, MA. This full-time role offers flexible scheduling and work-life balance, with visits to patient homes in Brockton and surrounding towns.
Prior home health experience is not required, and you will join a multi-disciplinary team delivering skilled nursing and rehab services.
Qualifications
- Massachusetts license to practice physical therapy.
- 1 year of clinical PT experience preferred.
- Ability to travel to patient locations and work in homes.
Responsibilities
- Make home visits to clients.
- Perform mobility and strength assessments.
- Develop and implement individualized PT care plans.
- Educate clients and family members on therapeutic activities.
- Document observations and interventions accurately.
