Job summary

One Home Health is seeking an Experienced Home Health Physical Therapist (PT) to deliver in-home therapy in Elmhurst and surrounding communities. The role offers flexible assignments including Full-Time, Part-Time, and PRN/Per Diem with compensation ranging from $75.00 to $85.00 per hour based on experience and clinical expertise.

Qualifications

• Active Illinois Physical Therapist (PT) license.
• Graduate of an accredited Physical Therapy program.
• Current CPR/BLS certification.
• Valid driver’s license and reliable transportation.
• Excellent communication, organization, and time-management skills.
• Ability to work independently in a home health setting.

Responsibilities

• Perform comprehensive physical therapy evaluations in patients' homes.
• Develop and implement individualized treatment plans to improve mobility, strength, balance, and functional independence.
• Provide skilled physical therapy interventions according to physician orders and established care plans.
• Assess patients' home environments and recommend safety modifications and assistive devices as appropriate.
• Educate patients and caregivers on exercises, injury prevention, fall prevention, and home safety.
• Collaborate with physicians, nurses, occupational therapists, speech therapists, and other members of the interdisciplinary care team.
• Monitor patient progress and adjust treatment plans based on clinical findings.
• Complete accurate, timely, and compliant clinical documentation.
• Participate in discharge planning and patient outcome evaluations.
