Job summary

Optum's DFW Home Health is hiring a full-time Physical Therapist / DPT to serve Weatherford, Springtown and Azle, TX. This role includes a $10,000 bonus for full-time employees and relocation assistance.

You will perform assessments, develop therapy plans, implement interventions to improve mobility and reduce pain, and coordinate with the care team and families to support patient recovery.

Qualifications

• Current Physical Therapy licensure in the state of practice.
• Current CPR certification.
• Access to a dependable vehicle or reliable transportation.
• Ability to function in home health settings with diverse patient populations.

Responsibilities

• Evaluates the patient’s functional status and physical therapy needs.
• Assists in developing the therapy plan of care.
• Provides services within the scope of practice per state laws.
• Observes and reports patient response to treatment and changes in condition.
• Treats patients to relieve pain and restore function.
• Instructs patients and families about plan of care and progress.
• Completes evaluations and documents progress notes per policy.
• Reports outcomes and anticipated treatment frequency.
