Qualifications

• 1 year of physical therapy experience in home health is required.
• State licensure as a Physical Therapist is required.
• CPR certification is required.
• Adult and geriatric patient populations served.
• Home Care Home Base EMR training will be provided if needed.

Responsibilities

• Provide PT home health visits in the Moraine/Dayton area on a 5x8 schedule.
• Perform day shifts, 8 hours per day, Monday to Friday, with possible weekend visits.
• Coordinate care with families and caregivers and document progress.
