Job summary

Aveanna Healthcare is seeking a Physical Therapist (PT) for Home Health in the Venice/Nokomis/Sarasota area. You will assess, teach, and deliver PT interventions aligned with the physician and home health plan of care, collaborating with an interdisciplinary team.

Full-time schedule Monday–Friday with rotating weekends. You will conduct initial and ongoing evaluations, document progress, and participate in discharge planning with patient and family, ensuring quality outcomes in the local

Qualifications

• Degree in Physical Therapy from an accredited program.
• Current, unrestricted state license as a Physical Therapist.
• Proficiency in clinical assessments, documentation, and policy compliance.
• Valid CPR and driver's license.

Responsibilities

• Perform initial and ongoing mobility, coordination, strength assessments and functional evaluation.
• Provide written evaluation and plan of care defining PT needs, goals and outcomes.
• Deliver physical therapy services per physician orders and home health plan of care.
• Maintain plan of care, document progress, adjust goals, and plan discharge.
• Coordinate with health care team and family; educate on home exercise program.
• Supervise PTA/HHA as applicable and complete OASIS assessments where allowed.
