Phoenix Home Care in Topeka, KS is seeking licensed Physical Therapists to join our home health team. We offer both full-time and part-time opportunities, a sign-on bonus, and a competitive pay range.

You'll provide PT services in patients' homes, collaborate with a multidisciplinary team, and ensure compliance with ethics and state regulations while maintaining work-life balance through AI-assisted charting.

Formación

Active license in Kansas required.
Home Health experience preferred, not required.

Responsabilidades

Provides PT assigned services to patients in home setting under physician plan of care and supervision of a licensed PT.
Participates in communication and case conferences to maximize continuity and quality of patient care.
Exhibits sound judgement and technical skills to perform treatments per agency protocols.
Maintains compliance with Agency Code of Ethics.
Complies with agency standards, policies, state/federal regulations, and payer requirements.
Practices safety, infection control and environmental measures.
Performs miscellaneous duties as assigned by Supervisor.
