Job summary

Capital Region Home Health, part of LHC Group and Optum family, seeks a dedicated Physical Therapist to evaluate patients in home health, determine care needs, and implement treatment plans to improve mobility and function. You will document progress, coordinate with nurses and physicians, and educate patients and families about the plan of care.

Qualifications

• Active Physical Therapy licensure in the state of practice.
• Current CPR certification.
• Reliable vehicle or access to transportation.
• Ability to work with diverse patient populations in home settings.

Responsibilities

• Evaluate patient functional status and plan care.
• Deliver PT services per plan and state guidelines.
• Monitor patient progress and document progress notes.
• Coordinate with care team and inform family/caregivers.
• Complete plan of care and reports per policy.
