Adoration Home Health in Covington, GA is seeking a compassionate Physical Therapist to join our home health team. You will deliver patient‑centered care, evaluate and treat per physician orders, and collaborate with a supportive treatment group to maximize independence and function.

This full‑time role offers a $20K sign‑on for external candidates, weekday visits, and a chance to grow with a growing network of care.

Qualifications

• Graduate of a CAPTE-approved Physical Therapist education program.
• Current state PT licensure and CPR certification; CPR provided on hire.
• Two years of adult PT experience, home health preferred.
• Excellent observation, communication, problem solving, and basic math skills.

Responsibilities

• Perform PT evaluation and treatment per physician orders, including adaptive equipment.
• Establish measurable goals and develop treatment plans to maximize independence.
• Maintain communication among treatment team, physician, patient, and family.
• Document services thoroughly for proper billing.
• Supervise PT Assistants and PT Aides in line with state practice act.

Responsibilities

As a Home Health Physical Therapist, You will:

• Perform physical therapy evaluation and treatment of patients based upon physician orders, including recommendation and procurement of adaptive equipment
• Establish measurable goals and develop treatment plans to move patients toward their maximum level of independence and/or function
• Maintain communication among the treatment team, physician, patient, and family members as applicable
• Maintain thorough and timely documentation of services provided to ensure proper billing
• Supervise Physical Therapist Assistants and Physical Therapy Aides in direct patient care and patient-related activities in accordance with state practice act

Qualifications

• Graduate of a Physical Therapist Education program approved by CAPTE
• Current Physical Therapist licensure in State and American Heart Association CPR certification required; CPR certification may be offered and completed upon hire
• Two (2) years of experience in adult physical therapy, home health experience preferred
• Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; physical therapy skills per competency checklist
