Job summary

A local healthcare provider in Indiana is seeking a Home Health – Physical Therapist (RPT) to provide care and rehabilitation to geriatric patients at home. The ideal candidate has strong interpersonal skills and attention to detail. Responsibilities include patient evaluations and documentation of treatment plans. Qualifications

• Experience as a Physical Therapist preferred but not required; new grads welcome.
• Must be currently licensed to practice in Indiana.
• Must be CPR certified or willing to acquire certification.

Responsibilities

• Provide rehabilitation treatment and care in the home to the geriatric population.
• Evaluate patients and develop rehabilitation plans.
• Document patient progress and communicate with other clinicians.
